The project works with isolated and vulnerable older people to develop and implement personal lifestyle plans, focusing on improving physical activity levels, and improving weight management and nutrition knowledge.

Participants access the programme mainly via the Age Concern Halton Participation Organiser, who will work with sheltered accommodation providers, registered social landlords, and existing Age Concern networks. The Participation Organiser introduces them to the Lifestyle Co-ordinator, who then works with the participant to identify their key health issues, and to develop a lifestyle plan. The Lifestyle Advisor will form a relationship with the participant, providing advice, written diet and exercise programmes, opportunities for group classes, and signposting to other community interventions, which includes other projects funded by Target: Wellbeing.

This will be delivered as part of a 10-week intensive programme which will follow on from an initial 1:1. This will be followed by monthly 1:1s over a 12-month period. After this period they will be reassessed for on going support. Longer term support to monitor and maintain the changes sought by the lifestyle plan will be through the Participation Organiser who will maintain contact with the client. The project will contribute to the costs and accommodation of both the Lifestyle Advisor and Participation Organiser.

Outcomes: 
To work with socially isolated and vulnerable older people to develop and implement personal lifestyle plans in order to improve self-management; focusing on improving physical activity levels and improving weight management and nutrition knowledge. By recruiting volunteers on to the programme via Age Concern - use a peer to peer approach to increase the likelihood of engagement instead of the professional: client approach. The client will form a relationship with the lifestyle advisor; providing advice, written diet and exercise programmes, opportunities for group classes and signposting to other community interventions. This will contribute towards improvements in healthy eating, physical activity and mental well-being.
We set up the project because: 
Age Concern Mid-Mersey has assessed the needs of older people in Halton using a participatory approach. As part of the Older People’s Action Plan, which was developed in consultation with older people, physical activity and specific nutrition advice has been highlighted as a significant gap in service history. The project contributes towards the management of the impact of chronic or long-term conditions through the application of preventative approaches. The project will cut through the themes of supporting people with predominantly sedentary lifestyles and supporting people at risk or who are already suffering from diet related illnesses by improving their knowledge and skills.
