Halton
Halton’s Target: Well-being programme focuses on the super output areas within the wards of Castlefields, Riverside, Windmill Hill, Ditton, Grange, Halton Brook, Mersey, Broadheath, Norton North and South and Appleton and benefits older people, people with mild to moderate depression, hard to reach groups, victims of domestic violence, people with long term conditions who are at risk of developing additional co-morbidities, people living with significant levels of deprivation, young people, sedentary people, schools and local businesses and people suffering from cancer.
The Target: Wellbeing projects are aligned to Halton’s Local Area Agreement and the Community Strategy 2006-2011 and the objectives are to achieve either some or all of the following:
To understand fully the causes of ill health in Halton and act to improve the overall health and wellbeing of local people
To reduce the burden of disease by concentrating on lowering the rates of cancer and heart disease, mental ill health and diabetes and addressing the health needs of local people
To promote a healthy living environment and lifestyles to protect the health of the public, sustain individual good health and wellbeing and help prevent and efficiently manage illness
To remove the barriers that disable people and contribute to poor health through ensuring that people have ready access to a wide range of social, community and housing services, and cultural and sporting activities that enhance their quality of life
To lay firm foundations for a healthy start in life and support those most in need in the community by maximising and promoting autonomy
The programme benefits from over £400k lottery funding from BIG and £150k match funding provided by a variety of local community and voluntary organisations, the Borough Council and the Primary Care Trust.
